Master Assignment Sheet |
Program faculty may use this blank form to indicate the specific SCAN proficiencies assigned in each rotation or evaluation period. Employers may use the form to indicate the specific competencies that will be considered during an employee evaluation cycle. |

Master Proficiency List (specialty specific) |
This list documents the date(s) when a student has achieved, and maintained, entry-level competency on all performance objectives within a proficiency. This form provides a ready reference for faculty or employers who can quickly ascertain which competencies have been completed and which remain to be done. |

Performance Objectives Overview (specialty specific) |
This chart (one for each specialty) provides a quick review of the ratings (or grades), obtained by a student, for the performance objectives within each proficiency. This provides a more detailed appraisal of individual competencies than does the master proficiency list. |

Clinical Evaluation Form |
This form is used to provide narrative comments related to the student's performance. The form may also be used by employers for sonographer evaluation. |

Continuing Education Record |
This form allows an employer to provide a documentation record for each sonographer's Continuing Education. This is one way to ensure clinical competence and compliance with CME requirements in the maintenance of credentials. This is an employer use form and will not be applicable for the student in a sonography program unless the program requires the student to attain CME credit as part of the curriculum. |

Clinical Examination Totals (specialty specific) |
This form is used to document the student's monthly number of cases scanned. Totals are useful to the program faculty to ensure that the student is afforded appropriate opportunity at the clinical site to scan and that the student is meeting the requirements stipulated by the program for clinical competence for the period specified in the Master Assignment Sheet. (Forms that are not applicable to your program may be discarded.) The totals are compiled from the Clinical Logbook and entered in the appropriate area on the Clinical Examination Total form. It is the student's responsibility to track the number and type of cases that he/she participates in during the clinical day. This type of record keeping allows the student to develop the habit of appropriate documentation of cases performed that should carry over into the workplace after completion of the program. The Clinical Examination Totals form may also be utilizedby a department head or chief sonographer to document the total number and type of exams done in a month. |
